Get a referral
If you don’t yet have a referral from a GP or a specialist, you will need to organise this referral before booking an appointment with Dr Chuah.
Step 2
Book an appointment
Once we have received a referral, we can book an appointment for you to see Dr Teong Chuah. Give us a call today to do so. At your appointment, Dr Chuah will discuss your case with you, outline options, and recommendations, and answer your questions.
Step 4
Tests or surgery
From here, the appropriate tests or surgery will be scheduled to take place.

This is a weight loss surgery that involves removing a portion of the stomach to create a smaller, sleeve-shaped stomach. This means that patients can’t eat as much, and can help patients feel full more quickly and consume fewer calories.
Private health care information
If you need to locate Dr Teong Chuah’s information for your health care rebate, you should be able to find this information on your invoice or receipt. You will need Dr Chuah’s practitioner number, as well as the amount charged, the item codes for each part of your surgery/consultation, and other details.
